In accordance with part 2 of Article 37 of the Federal Law of July 13, 2015 N 220-FZ "On the organization of regular transportation of passengers and luggage by car and urban ground electric transport in Russian Federation and on amendments to certain legislative acts of the Russian Federation" (Sobraniye zakonodatelstva Rossiyskoy Federatsii, 2015, N 29 (part 1), art. 4346) I order:
1. Approve the attached form of quarterly reports on the implementation of regular transportation.
2. Establish that a report on the implementation of regular transportation is submitted quarterly no later than the 15th day of the month following the reporting quarter.

ABOUT REGULAR TRANSPORTATION

1. Columns 3 - 7 of paragraph 1 indicate the total number of flights for the reporting period, calculated on the basis of the established schedule.
2. Columns 3 - 7 of paragraph 2 shall indicate the total number of flights performed and flights interrupted as a result of breakdowns of vehicles and completed by vehicles sent to replace them during the reporting period.
3. Paragraph 3 shall indicate information on unperformed flights, including those interrupted and not completed as a result of a breakdown of vehicles, by days of the reporting period:
in column 2 - date (day, month, year);
in column 3 - route number;
in columns 4 - 8 - the number of unfulfilled flights.

The form and procedure for sending quarterly reports on regular transportation has been approved.
Article 37 federal law dated July 13, 2015 N 220-FZ "On the organization of regular transportation of passengers and luggage by road and urban ground electric transport in the Russian Federation and on amendments to certain legislative acts of the Russian Federation" a legal entity, an individual entrepreneur, an authorized participant in a simple partnership agreement, with by which the state or municipal contract or who have been issued a certificate of transportation on a regular transportation route, are obliged to send to the authorized federal executive body, the executive body of the constituent entity of the Russian Federation, the authorized local government body that concluded this contract or issued this certificate, quarterly reports on the implementation of regular transportation.
The order establishes that a report on the implementation of transportation should be submitted quarterly, no later than the 15th day of the month following the reporting quarter. Such a report must include, in particular:
- the name of the legal entity (full name of the individual entrepreneur) submitting the report;
- the number of flights in accordance with the established schedule;
- the number of actually performed flights;
- information about canceled flights.

Guidelines for Completing the Quarterly Scheduled Operation Report Form
1. Columns 3-7 of paragraph 1 indicate the total number of flights for the reporting period, calculated on the basis of the established schedule.
2. Columns 3-7 of paragraph 2 indicate the total number of flights performed and flights interrupted as a result of vehicle breakdowns and completed by vehicles sent to replace them during the reporting period.
3. Paragraph 3 shall indicate information on unperformed flights, including those interrupted and incomplete as a result of a breakdown of vehicles, by days of the reporting period:
